2.5×3.5cm (197×276px at 200 DPI) photo, white background, per NSDL/Protean guidance — this is the more commonly used of two slightly different official variants (UTIITSL specifies 213×213px at 300 DPI instead). Confirm which processing agency your application uses before submitting.
